Leadership Alignment & System Transformation
The tragedy of the modern health system is the distance between the brilliance of its people and the fragmentation of its structures. Every system is full of committed professionals working tirelessly inside frameworks that no longer fit the moment. Strategy lives in the boardroom. Operations live in the trenches. Finance, policy, and the human experience drift apart. Effort remains high, but momentum stalls.
Coherence+ is the work of closing that distance. It is the discipline of bringing the disparate pieces back into a singular orbit: The vision, narrative, operations, and lived experience so systems move with intention rather than reaction.
We work with leaders navigating high-consequence complexity and institutional pressure. We align priorities, clarify impossible tradeoffs, and architect systems that eliminate disparities, strengthen access, and drive growth—from now into the future.
Coherence+ is delivered through signature instruments, most notably the Olusan Service Line Design. This is a structured, decision-forcing framework that integrates access, capacity, finance, and governance into a single, board-ready architecture. Our designs replace disconnected plans and parallel workstreams with a durable system that leaders can actually run.

THE FOUNDER
After nearly two decades leading hospital operations, Tosan recognized that the most persistent challenges in healthcare are not failures of effort or expertise, but failures of system design. His career inside complex public institutions became the proving ground for what is now Olusan.
Through Olusan Platforms, Tosan builds governance-grade systems that translate insight into alignment and then alignment into durable impact. ContinuityOS, Olusan’s flagship platform, provides the infrastructure to ensure continuity of care attains high reliability even as policy and operational conditions change.
Tosan remains deeply embedded in the governance of the field. He serves as an elected Board Member and Governance Chair of the Institute for Healthcare Improvement (IHI) and has led system-wide transformation efforts spanning hospital operations, public health infrastructure, and safety-net care delivery.
Tosan has been recognized with 40 Under 40 honors from Modern Healthcare, Becker’s Hospital Review, and the San Francisco Business Times, along with community impact awards from the NAACP, Native American Health, San Francisco Department of Public Health, City of Oakland, California Legislative Assembly, and Special Congressional Recognition.
His work is grounded in a singular objective: to move healthcare and public institutions from reaction to intention and to build resilient systems that continuously improve outcomes over time.
